I (Participant and/or minor), hereby acknowledge that I am voluntarily participating in activities organized by Combat to Kayaks, a nonprofit organization (hereinafter referred to as "CTK"). I understand that these activities may include, but are not limited to, kayaking trips, kids fishing events, and charity events involving rough terrain, rough waters, and fishing out of kayaks. In consideration for being allowed to participate in these activities, I hereby agree to the following: Assumption of Risks: I understand that the activities organized by CTK involve certain inherent risks, including but not limited to, the risks associated with water-based activities, rough terrain, unpredictable weather conditions, and the use of equipment. I acknowledge that these activities may be physically demanding and that there are inherent risks of injury or property damage. Health and Fitness: I hereby affirm that I am in good health and physical condition to participate in the activities organized by CTK. I understand that it is my responsibility to inform CTK of any pre-existing medical conditions, physical limitations, or any other factors that may affect my ability to participate safely. Release of Liability: I hereby release, discharge, and hold harmless Combat to Kayaks, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any damages, injuries, or losses that may occur as a result of my participation in CTK activities, including those caused by the negligence of CTK or its representatives. Medical Treatment Authorization: In the event of injury or illness during my participation in CTK activities, I authorize CTK to secure and consent to medical treatment deemed necessary by qualified medical personnel. I understand that CTK will make reasonable efforts to contact emergency contacts listed on my registration form. Photographic Release: I grant CTK the right to take photographs and/or videos of me during the activities, and I hereby consent to the use of these materials for promotional, advertising, or other purposes without compensation. I have read this Liability Waiver and Release Form, and I understand its contents. I am aware that by accepting this document, I am waiving certain legal rights, including the right to sue. I voluntarily agree to be bound by its terms.
